Tennessee Prison Chief Resigns After Botched Execution Leaves Inmate on Ventilator

Tennessee Department of Correction Commissioner Frank Strada is stepping down this month following the deeply troubling botched execution of Christa Pike, who was left hospitalized and on a ventilator after officials struggled for an extended period to find a vein and administer the lethal injection. Governor Bill Lee announced Strada’s resignation, stating it was in the “best interest of Tennesseans” as former U.S. Attorney Ed Stanton leads an independent review into the execution protocol failures. Pike, convicted of the 1995 murder of Colleen Slemmer, reportedly cried out and whimpered during the prolonged procedure, which involved multiple needles and the eventual bursting of a vein, leading to her transfer to a hospital where she required intubation. This incident has prompted Governor Lee to halt all remaining executions scheduled for this year pending the review’s findings, a review that will scrutinize whether the state’s protocol was followed and what systemic issues may have contributed to the catastrophic failure. Pike’s legal team has characterized the event as a “systemic failure” that demands a broader examination of Tennessee’s entire death penalty system, especially since Stanton previously led a review that resulted in significant revisions to the state’s lethal injection protocol in 2022.
